Validation Documentation: Evidence of Quality Innovation Network-Quality Improvement Organization (QIN-QIO) relationship to implement self-management training programs. Include the following element:
1) QIN-QIO engagement – Documentation from QIN-QIO of eligible clinician or group’s engagement and use of services (e.g., email exchange, participation letter, listed on QIN-QIO website as partner) to assist with participation in self-management training program(s) such as the Diabetes Self-Management Program (DSMP).
